Applications for the Mod Rehab program can be filled out at the HACF main office located at 209 Madison St., Frederick, MD 21701 on Tuesdays from 9:00 AM to 2:45 PM.
Everyone who applies for a program must apply when the program's waiting list is open. The Housing Authority of the City of Frederick (HACF) must give public notice of when a waiting list is open and where applications are being accepted. The HACF advertises when the waiting list is open in local newspaper, radio and on this Web site.
